Skokie Institute of Allied Health and Technology follows the following admission policy for students to be admitted to their respective programs of interest. The student is required to fill out an online application, and provide (email, fax, upload) the ID/DL, High school diploma/GED, if needed transcripts to show high school graduation and if working provide with a copy of their resume.

SIAHT doesn’t discriminate based on gender, sexual preference, race, color, religion, marital status or national or ethnic origin. The policies are enforced by federal law under TITLE X of the Educational Amendments of 1972; TITLE VI of Civil Rights Act of 1964. The confidentiality of student records is maintained under the Family Educational Rights and Privacy Act of 1974.

ADMISSIONS REQUIREMENTS AND PROCEDURE:

     Applicants must meet the following guidelines for admission.

  • Interested applicant contacts the school representative via email through SIAHT website https://www.siaht.org/ or via phone: (847)461-3985

  • Applicants can fill out the form via online and submit the required documents.

  • All applicants for training and educational classes and programs must be 18 years of age or older.

  • Applicant must fill out an application form.

  • Applicants must provide a copy of GED or high school diploma

  • Applicants must provide with a copy of curriculum vitae or resume that verifies their work experience and education mentioned in the application form.

  • Students who don’t have work experience need to show their GED/high school diploma.

  • Students  once complete the registration form are contacted by admissions team and directed to fill out the Enrollment agreement. The enrollment agreement is a requirement as it explains our responsibility towards the student as an institution, outlines our rules/ expectations and gives details about a student responsibility and expectations as a student. 

 

 

ADMISSIONS BASED ON PROGRAMS REQUIREMENT:

  • The fitness trainer program requires students to have a GED/high school diploma and the student has to complete a non-numerated externship in a gym selected for them by the institution.  The students are provided with CPR certification before they are able to start their externship.

  • The nutrition wellness coach program requires students to have GED/High school diploma. The student should be able to show work experience in the field of Nutrition or Fitness.  The student is required to complete case studies in the last term. The students are provided with CPR certification.

  • The medical billing and insurance specialist program requires students to have a GED/high school diploma. The student has to complete a project writing in the last term.

  • The pharmacy technician program requires students to have a GED/high school diploma and a background check per the requirement of Walgreens where the student completes the externship. Students are also provided with CPR certification before they are ready to start their externship.

  • The neurophysiology program requires students to have an Associates/Bachelor’s in Science with proof of work experience in the health care field. The student is responsible for securing their own clinical site, the institution will help them in this case as needed. Interested Prospects have to contact the Executive Director she then sets an interview with Dr. Wasif.  Before a student is accepted he/she is interviewed by Dr. Wasif Sheikh the MD of the program. If he accepts the student the student is then asked to complete the registration package.
